- Partial Conditional – Submitted by subcontractor with interim monthly partial pay request applications along with the sworn statement
- Partial Unconditional – Two copies will be enclosed with check. Signed copies must be returned to Moyle Construction before release of future payments.
- Full Conditional – Sent prior to issuing of final payment. Waives subcontractor’s lien rights against the project. Final payment will not be mailed until this is received.
- Full Unconditional – Two copies will be enclosed with check. Signed copy must be returned to Moyle Construction.
- Sworn Statement – Must accompany each monthly bill and list all major vendors and subcontractors.
